15-year history worksheet
Some posts follow an administrative processing 221(g) with a request for 15 years of personal history and a CV. If your field is technical or scientific, prepare this before the interview: reconstructing 15 years of trips from memory after a refusal is slow, and it is where inconsistencies with your DS-260 creep in.
Based on one reported request, from another post
The items below follow a single form an applicant's spouse posted publicly in September 2026, from an interview outside London. We have not seen London issue it. Most of it matches Form DS-5535. See when administrative processing asks for your history for the full account, and which careers draw technology checks.
How to fill it in
- Answer every item. Write "none" where nothing applies: a blank reads as an omission.
- Check each section against your DS-260. Where the DS-260 was shorter, add to it rather than contradicting it. Tick the box when you have checked.
- Describe your work accurately and plainly. Do not overstate or understate it: misrepresentation is far worse than a delay.
- Your answers stay in this browser only.
Your email
Use the subject line exactly as the post gives it. The one below is the format reported; send to the address on your sheet or in the post's instructions, not a guessed one. Attach your CV. Send it once, then wait.
